How long do you keep medical records? |
|
| Adult medical
records are kept for 10 years. Records of patients under the age 18 at time of
treatment are kept 10 years past the last date of visit, or the patient’s
current age of 21. (whichever is greater) |

What are your hours of operation? |

| We are open
Monday through Friday 7am to 5pm Central Standard Time. We are closed on major
national holidays. |
Is there are charge for copies? |
|
Yes. Patients
are charged a $3.20 copy fee and $.28 per page.
Important: There is no charge for records used for continuity of patient care.
Examples of this are records going to another medical facility or doctor’s
office. |
When can I expect
to receive my copy?

| Because of
the large number of requests we receive daily, it may take up to 10 work days
to fulfill your request. |
Can you fax the copies
to my home or office? |

| No. We can
not fax medical records to your home or office due to confidentiality issues. |
Can you send my medical records to my other health care providers? I.e. Another
medical facility or doctor’s office. |

| Yes. These
types of requests are our first priority. |
Can my spouse request and receive copies of my medical record? |
|
| No. He or she
may not receive copies of your medical record without a consent signed by you
authorizing Bay Area Medical Center to release copies to him/her. |
May I obtain a copy of my x-ray films from you? |

No. Please contact the Radiology file room at 715-735-4200 ext.3288. |
How do I request a copy of my birth certificate or child’s birth certificate? |

Center does not keep birth certificates for patients born at the medical center.
You may request copies from the Marinette County Register of Deeds at 715-732-7553
or visit their website at www.marinettecounty.com |
How may I request copies from my last doctor visit? |

Contact your physician’s office for assistance. We do not keep private physician
office records. |
Can you explain charges on my bill? |

| No. Please
contact our billing customer service department at 715-735-4203. Their office
hours are Monday through Friday 8am to 4:30pm Central Standard Time. |
How do I get medical records from the former Oconto Memorial Hospital? |

As per PUBLIC NOTICE: Effective August
1, 2003. The former Oconto Memorial Hospital, 405 First Street, Oconto, WI.
Permanent record storage is being
maintained by a professional record storage company: Sourcecorp Record Center.
Medical Record Requests can be made by contacting the release of information technician
by phone at 920-469-5000 ext.3772 or 800-469-0689. Fax request at: 920-469-0689
or written requests at: 1030 Ontario Road, P.O. Box 8408, Green Bay, WI 54308-8408.
Charges for these requests will be handled on a case-by case basis with each requester. Permanent radiology film storage
is being maintained by Bellin Health / Oconto.
