Often a stay in the hospital means a disruption in your life. Schedules become difficult. Questions about finances are complex. You may feel anxious and concerned about what to do after you are discharged.

At Bay Area Medical Center, our concern for you includes your continuing care after leaving our hospital. We want to be sure that you are discharged into the safest possible environment, while maintaining the greatest degree of independence. Our case managers and social workers will not only assist you in planning and coordinating your post-hospital arrangements, but also will help answer questions about finances, special programs and services. Case Management Service hours at BAMC are Monday through Friday, 7 a.m. until 4:30 p.m.
